L\u2019\u00e9quipe de CAREX a class\u00e9 ces agents comme des sources d\u2019exposition importantes dans le contexte canadien. Dans le cadre de son effort continu de surveillance de ces expositions, l\u2019\u00e9quipe de CAREX suit \u00e9galement des substances qui pr\u00e9occupent de plus en plus les Canadiens. Parmi celles-ci, il y a des substances que le CIRC pr\u00e9voyait \u00e9valuer dans le futur qui pourraient \u00e9ventuellement \u00eatre class\u00e9s comme des agents carcinog\u00e8nes connus ou pr\u00e9sum\u00e9s. Diff\u00e9rents mat\u00e9riaux aux propri\u00e9t\u00e9s vari\u00e9es sont utilis\u00e9s dans l\u2019impression 3D, notamment les thermoplastiques (les plus courants \u00e9tant l\u2019acrylonitrile butadi\u00e8ne styr\u00e8ne (ABS) et l\u2019acide polylactique), les m\u00e9taux, la c\u00e9ramique et le verre, les r\u00e9sines et les mat\u00e9riaux flexibles (tels que le polypropyl\u00e8ne, le caoutchouc) . Typiquement, le mat\u00e9riau est d\u00e9pos\u00e9 sous sa forme liquide ou en poudre. Compte tenu de son potentiel de rationalisation de la production (par exemple dans les secteurs de la fabrication, de l&rsquo;a\u00e9rospatiale et des soins de sant\u00e9) et de son accessibilit\u00e9 croissante, l&rsquo;impression 3D est de plus en plus utilis\u00e9e par l&rsquo;industrie et le grand public. Cela pourrait avoir des cons\u00e9quences sur l&rsquo;exposition professionnelle et environnementale. Au cours des derni\u00e8res ann\u00e9es, l\u2019\u00e9valuation de cette exposition a suscit\u00e9 un int\u00e9r\u00eat accru. Des \u00e9tudes pr\u00e9liminaires montrent que les imprimantes 3D peuvent produire des niveaux \u00e9lev\u00e9s de particules ultrafines, qui auraient un impact sur le fonctionnement respiratoire et les r\u00e9sultats cardiovasculaires. Ils peuvent \u00e9galement produire des compos\u00e9s organiques volatils (COV), qui englobent un grand groupe de compos\u00e9s impliqu\u00e9s dans l&rsquo;irritation des yeux et du nez et dans les l\u00e9sions du foie, des reins ou du syst\u00e8me nerveux central. En particulier, le styr\u00e8ne, potentiellement canc\u00e9rog\u00e8ne, est le principal COV lib\u00e9r\u00e9 lors de l\u2019utilisation de l\u2019ABS. Le BPA sert aussi pour les re\u00e7us en papier et des articles en plastique comme les bouteilles de boisson et les contenants d\u2019aliments. L\u2019Organisation mondiale de la sant\u00e9 a examin\u00e9 la canc\u00e9rog\u00e9nicit\u00e9 du BPA en 2010 et a conclu qu\u2019il n\u2019y avait pas assez de preuve pour en \u00e9valuer le potentiel canc\u00e9rog\u00e8ne \u00e0 ce moment-l\u00e0. En cons\u00e9quence de l\u2019\u00e9clairage artificiel, les p\u00e9riodes de clart\u00e9 et de noirceur ne d\u00e9pendent plus seulement du soleil. Le recours \u00e0 l\u2019\u00e9clairage avec DEL (diodes \u00e9lectroluminescentes) (p. ex. pour l\u2019\u00e9clairage de rue et l\u2019\u00e9clairage int\u00e9rieur) et les appareils \u00e9lectroniques \u00e9mettant de la lumi\u00e8re (p. ex. les t\u00e9l\u00e9s, t\u00e9l\u00e9phones cellulaires, ordinateurs et livres num\u00e9riques) a s\u00e9rieusement augment\u00e9 l\u2019exposition de la population \u00e0 la lumi\u00e8re bleue. Peu importe laquelle, toute exposition \u00e0 une lumi\u00e8re la nuit a un effet perturbateur sur le rythme circadien de l\u2019organisme, la lumi\u00e8re bleue causant la plus grande perturbation. Le rythme circadien est un processus biologique qui r\u00e8gle le cycle \u00e9veil-sommeil et le cycle solaire clart\u00e9-noirceur.<\/p>\n<p>En perturbant le rythme circadien, la clart\u00e9 \u00e9mise pendant la nuit r\u00e9duit la production de m\u00e9latonine. On sait que les courtes longueurs d\u2019onde de la lumi\u00e8re visible (lumi\u00e8re bleue) ont le plus gros effet sur la suppression de la m\u00e9latonine. La m\u00e9latonine est l\u2019hormone qui fait passer d\u2019abord des yeux au cerveau l\u2019information sur la lumi\u00e8re ambiante et la noirceur, puis au reste de l\u2019organisme. Les chercheurs conjecturent que la suppression de la m\u00e9latonine pourrait \u00eatre associ\u00e9e \u00e0 un plus grand risque de cancer du sein et de cancer de la prostate.<\/p>\n<p>Plusieurs \u00e9tudes d\u00e9montrent le lien entre le d\u00e9r\u00e8glement du rythme circadien au travail et un plus gros risque de cancer du sein et de la prostate. CHIMIQUES<\/strong><\/p>\n<p>Les Canadiens sont expos\u00e9s \u00e0 un m\u00e9lange de substances chimiques, naturelles ou artificielles, \u00e0 leurs lieux de travail et dans leurs communaut\u00e9s. Une fois qu\u2019elles sont l\u2019environnement, ces substances peuvent p\u00e9n\u00e9trer dans le corps humain par un nombre de voies et moyens d\u2019exposition, incluant l\u2019inhalation de l\u2019air ambiant et ext\u00e9rieur, l\u2019ingestion de nourriture et d\u2019eau et l\u2019utilisation de produits de consommation. Il est difficile de caract\u00e9riser de mani\u00e8re pr\u00e9cise ces expositions complexes et d\u2019en mesurer l\u2019impact sur la sant\u00e9 humaine, surtout en combinaison les unes avec les autres. Certains scientifiques conjecturent que les m\u00e9langes de substances chimiques peuvent avoir un effet canc\u00e9rog\u00e8ne m\u00eame si les \u00e9l\u00e9ments constituants ne sont pas des agents canc\u00e9rog\u00e8nes connus. La recherche actuelle se penche principalement sur l\u2019effet sur la sant\u00e9 de l\u2019exposition aux substances chimiques, mais ce, une substance \u00e0 la fois, ce qui limite notre compr\u00e9hension de la mani\u00e8re dont ces substances fonctionnement en combinaison.<\/p>\n<p>Nombre d\u2019instituts et d\u2019organismes de r\u00e9glementation nationaux au Canada et \u00e0 l\u2019\u00e9tranger sont conscients des d\u00e9fis que ces m\u00e9langes de substances chimiques repr\u00e9sentent. L\u2019Organisation mondiale de la Sant\u00e9 (OMS) a pr\u00e9par\u00e9 un document d\u2019\u00e9valuation des risques des substances chimiques dans le but de soutenir l\u2019effort international et d\u2019arriver plus facilement \u00e0 une entente. L\u2019OMS a aussi d\u00e9fini plusieurs termes pertinents \u00e0 la discussion sur les m\u00e9langes chimiques. En voici un exemple :<\/p>\n<ul>\n<li><strong>Risque global<\/strong> &#8211; sc\u00e9nario d\u2019exposition qui tient compte d\u2019une substance p\u00e9n\u00e9trant par des voies multiples (aussi appel\u00e9\u00a0<i>une seule substance, toutes voies<\/i>).<\/li>\n<li><strong>Antagoniste<\/strong> \u2013 lorsque la toxicit\u00e9 de l\u2019exposition aux substances pr\u00e9sentes dans le m\u00e9lange est inf\u00e9rieure \u00e0 la toxicit\u00e9 anticip\u00e9e par la somme des parties.<\/li>\n<li><strong>Exposition cumulative<\/strong> \u2013 sc\u00e9nario d\u2019exposition qui tient compte de plusieurs substances et plusieurs voies de p\u00e9n\u00e9tration (aussi appel\u00e9\u00a0<i>multiples substances, toutes voies<\/i>).<\/li>\n<li><strong>Synerg\u00e9tique<\/strong> \u2013 lorsque la toxicit\u00e9 de l\u2019exposition aux substances pr\u00e9sentes dans le m\u00e9lange est sup\u00e9rieure \u00e0 la toxicit\u00e9 anticip\u00e9e par la somme des parties.<\/li>\n<\/ul>\n<p>Les chercheurs ont mis au point de nouvelles approches dans le but de mieux \u00e9valuer l\u2019exposition aux m\u00e9langes. Par exemple, certains ont utilis\u00e9 les r\u00e9sultats d\u2019\u00e9tudes de veille biologique \u00e0 grande \u00e9chelle pour d\u00e9terminer les coexpositions les plus courantes dans une population. Ces donn\u00e9es servent \u00e0 restreindre le champ des m\u00e9langes pouvant \u00eatre \u00e9tudi\u00e9s en priorit\u00e9 plus tard. D\u2019autres utilisent une mod\u00e9lisation math\u00e9matique et des bases de donn\u00e9es de d\u00e9pistage pour pr\u00e9dire si l\u2019exposition \u00e0 un m\u00e9lange produira un effet additif, synerg\u00e9tique ou antagoniste. Plusieurs nouvelles initiatives de recherche combinent la surveillance de l\u2019exposition avec la science de l\u2019exposition interne (p. ex. la g\u00e9nomique et la m\u00e9tabolomique) dans une approche appel\u00e9e exposomique qui vise \u00e0 caract\u00e9riser l\u2019exposition d\u2019une personne d\u2019une mani\u00e8re plus compl\u00e8te. On sait que ces mati\u00e8res ont un effet nocif pour la sant\u00e9, notamment le cancer et une alt\u00e9ration de la fonction de la glande thyro\u00efde, des poumons, et (ou) de l\u2019appareil reproducteur. Le\u00a0<a href=\"\/?p=9318\" target=\"_blank\" rel=\"noopener noreferrer\">cadmium<\/a>, les HAP et les BPC font partie des mati\u00e8res souvent lib\u00e9r\u00e9es pendant le recyclage de ces d\u00e9chets qui ont aussi \u00e9t\u00e9 class\u00e9es par le Centre International de Recherche sur le Cancer comme \u00e9tant\u00a0<i>canc\u00e9rog\u00e8nes pour l\u2019humain<\/i>\u00a0(Groupe 1).<\/p>\n<p>Compte tenu des conventions et r\u00e8gles internationales limitant l\u2019exportation de d\u00e9chets \u00e9lectriques et \u00e9lectroniques aux pays en voie de d\u00e9veloppement, ce secteur prend de l\u2019expansion au Canada. Des \u00e9tudes provenant de pays \u00e0 bas revenu ou \u00e0 revenu interm\u00e9diaire ont identifi\u00e9 l\u2019effet nocif sur la sant\u00e9 des personnes travaillant ou vivant pr\u00e8s de centres de traitement de d\u00e9chets \u00e9lectriques et \u00e9lectroniques. Deux \u00e9tudes canadiennes ciblent actuellement la sant\u00e9 et s\u00e9curit\u00e9 au travail de travailleurs d\u2019installations officielles de recyclage. Cette m\u00e9thode permet de fissurer la roche et d\u2019en extraire du p\u00e9trole ou du gaz.<\/p>\n<p>Plusieurs produits chimiques utilis\u00e9s dans la fracturation, dont l\u2019<a href=\"\/?p=8959\">ac\u00e9tald\u00e9hyde<\/a>, le\u00a0<a href=\"\/?p=9621\">naphtal\u00e8ne<\/a>\u00a0et la\u00a0<a href=\"\/project\/silica_(crystalline)\/\">silice cristalline<\/a>\u00a0(sable), ont \u00e9t\u00e9 class\u00e9s par le CIRC comme \u00e9tant des canc\u00e9rog\u00e8nes connus ou pr\u00e9sum\u00e9s. L\u2019exposition des travailleurs \u00e0 la silice au-dessus des niveaux admissibles d\u2019inhalation en milieu de travail a \u00e9t\u00e9 confirm\u00e9e. Les substances rejet\u00e9es dans l\u2019atmosph\u00e8re par l\u2019industrie du p\u00e9trole et du gaz comme <a href=\"\/project\/diesel_engine_exhaust\/\">l\u2019\u00e9chappement de moteur di\u00e9sel<\/a>, <a href=\"https:\/\/www.carexcanada.ca\/profile\/outdoor_air_pollution\/\">des mati\u00e8res particulaires<\/a>,\u00a0et les\u00a0<a href=\"\/project\/polycyclic_aromatic_hydrocarbons\/\">hydrocarbures aromatiques polycycliques (HAP)<\/a>, sont une autre source d\u2019exposition aux canc\u00e9rog\u00e8nes pendant la construction de plateformes d\u2019exploitation ainsi que pendant les activit\u00e9s de forage et de br\u00fblage \u00e0 la torche ou de ventilation. Ces agents chimiques sont lib\u00e9r\u00e9s dans l\u2019environnement pendant la fabrication et lorsque les produits qui les contiennent sont mis aux rebuts. Au fur et \u00e0 mesure que les produits se d\u00e9gradent, les EDPB demeurent pr\u00e9sents dans la poussi\u00e8re domestique o\u00f9 elle peut \u00eatre inhal\u00e9e et ing\u00e9r\u00e9e. Ils persistent dans l&rsquo;environnement, sont donc consid\u00e9r\u00e9es bioaccumulables, les polluants organiques persistants.<\/p>\n<p>Le CIRC a \u00e9valu\u00e9 une PBDE, d\u00e9ca-BDE et l&rsquo;a class\u00e9 comme groupe 3, non-classifiable quant \u00e0 sa canc\u00e9rog\u00e9nicit\u00e9 pour l&rsquo;homme. Aucun autre PBDE a \u00e9t\u00e9 examin\u00e9s pour son canc\u00e9rog\u00e9nicit\u00e9 potentielle. Toutefois, une commission d&rsquo;examen organiz\u00e9 par le National Toxicology Program des \u00c9tats-Unis, qui a examin\u00e9 un m\u00e9lange de six compos\u00e9s de PBDE, d\u00e9crit une preuve claire de l&rsquo;activit\u00e9 canc\u00e9rog\u00e8ne chez les rats et les souris m\u00e2les et femelles. La s\u00e9dentarit\u00e9 d\u2019un travail se mesure de plusieurs mani\u00e8res, \u00e0 savoir le temps pass\u00e9 assis, le titre du poste ainsi que des mesures directes tel qu\u2019un acc\u00e9l\u00e9rom\u00e8tre. Une m\u00e9ta-analyse de 2014 a d\u00e9montr\u00e9 un lien positif entre le fait de travailler assis et le cancer du c\u00f4lon. Une autre \u00e9tude a r\u00e9v\u00e9l\u00e9 que les participants qui avaient pass\u00e9 10 ans ou plus dans un travail s\u00e9dentaire pr\u00e9sentaient un risque deux fois plus grand d\u2019avoir un cancer rectal que ceux qui n\u2019avaient pas de travail s\u00e9dentaire. Les progr\u00e8s technologiques, notamment le perfectionnement des technologies d\u2019\u00e9clairage, ont permis l\u2019implantation d\u2019un nombre croissant de serres de culture. L\u2019\u00e9clairage horticole des serres de culture (aussi connu sous le nom d\u2019\u00e9clairage ou lampes de culture) \u00e9met les longueurs d\u2019onde dont se servent les plantes pour effectuer la photosynth\u00e8se (rayonnement de photosynth\u00e8se dont les longueurs d\u2019onde s\u2019\u00e9tendent de 400 \u00e0 700\u00a0nanom\u00e8tres [nm]). L\u2019\u00e9clairage horticole peut \u00e9galement servir \u00e0 bonifier la lumi\u00e8re naturelle dans les exploitations serricoles \u00e0 grande \u00e9chelle.<\/p>\n<p>L\u2019\u00e9clairage horticole conventionnel \u00e9met peu ou pas de <a href=\"https:\/\/www.carexcanada.ca\/profile\/artificial_uv_radiation\/\">rayonnement ultraviolet (UV)<\/a>. Cependant, les syst\u00e8mes d\u2019\u00e9clairage sont maintenant con\u00e7us pour \u00e9mettre plus de rayonnement\u00a0UV que l\u2019\u00e9clairage conventionnel. Le rayonnement\u00a0UV n\u2019est pas compris dans le spectre du rayonnement de photosynth\u00e8se, mais il est important biologiquement, car il aide \u00e0 r\u00e9guler la croissance des plantes.<\/p>\n<p>Le rayonnement\u00a0UV est un danger potentiel pour les travailleurs des serres de culture. Ce type de rayonnement dont la longueur d\u2019onde se situe entre 100 et 400\u00a0nm a \u00e9t\u00e9 class\u00e9 dans le groupe\u00a01 (<em>canc\u00e9rig\u00e8ne reconnu chez l\u2019homme<\/em>) par le Centre international de Recherche sur le Cancer. L\u2019exposition est associ\u00e9e au cancer de la peau et au m\u00e9lanome oculaire, et peut entra\u00eener d\u2019autres effets n\u00e9fastes pour la peau (par exemple, des br\u00fblures) et les yeux (par exemple, la photok\u00e9ratite). Les lampes\u00a0UV \u00e9mettent \u00e9galement du rayonnement infrarouge qui peut \u00eatre dommageable pour la peau et les yeux.<\/p>\n<p>Jusqu\u2019\u00e0 maintenant, on a men\u00e9 peu de recherches dans les serres de cultures afin de mesurer l\u2019exposition au rayonnement\u00a0UV en milieu de travail. Les recherches r\u00e9alis\u00e9es dans les serres de culture de cannabis r\u00e9v\u00e8lent qu\u2019il est possible que des travailleurs soient expos\u00e9s au rayonnement\u00a0UV provenant de l\u2019\u00e9clairage horticole. Cependant, les auteurs en viennent \u00e0 la conclusion que d\u2019autres recherches seront n\u00e9cessaires afin de caract\u00e9riser pleinement l\u2019exposition des travailleurs au rayonnement\u00a0UV, compte tenu, notamment, de l\u2019utilisation croissante de lampes de cultures \u00e9mettrices de ce type de rayonnement.<\/p>\n<p>Simpson (2018) a \u00e9galement publi\u00e9 qu\u2019on s\u2019est mis \u00e0 utiliser des lampes germicides dans des serres de culture de cannabis afin d\u2019aider \u00e0 lutter contre l\u2019o\u00efdium. Ce type de lampes \u00e9met beaucoup de rayonnement\u00a0UV, habituellement dans les longueurs d\u2019onde des UVC, qui sont les plus nocifs de tous les UV. On s\u2019attend \u00e0 ce qu\u2019ils soient plus fr\u00e9quents en raison des changements climatiques, les nouvelles conditions m\u00e9t\u00e9orologiques accentuant le risque de feux incontr\u00f4l\u00e9s\u00a0(3).<\/p>\n<p>Pendant la saison des feux, les taux \u00e9lev\u00e9s de <a href=\"https:\/\/www.carexcanada.ca\/profile\/outdoor_air_pollution\/\">particules fines<\/a> (PM<sub>2,5<\/sub>) contenues dans la fum\u00e9e sont le principal probl\u00e8me de sant\u00e9 publique. En plus des particules, la fum\u00e9e de feux incontr\u00f4l\u00e9s contient plusieurs canc\u00e9rog\u00e8nes connus et soup\u00e7onn\u00e9s, y compris le <a href=\"\/profile\/benzene\/\">benz\u00e8ne<\/a>, les <a href=\"\/profile\/polycyclic_aromatic_hydrocarbons\/\">hydrocarbures aromatiques polycycliques<\/a>, le <a href=\"\/profile\/13-butadiene\/\">buta\u20111,3\u2011di\u00e8ne<\/a>, l\u2019<a href=\"\/profile\/acrolein\/\">acrol\u00e9ine<\/a>, le <a href=\"\/profile\/formaldehyde\/\">formald\u00e9hyde<\/a> et d\u2019autres\u00a0(4). Elle contient \u00e9galement divers gaz et substances, y compris l\u2019ozone (O<sub>3<\/sub>), le monoxyde de carbone (CO), le dioxyde de soufre (SO<sub>2<\/sub>), des compos\u00e9s organiques volatils, le mercure et le dioxyde d\u2019azote (NO<sub>2<\/sub>)\u00a0(5).<\/p>\n<p>La fum\u00e9e de feux incontr\u00f4l\u00e9s peut causer des sympt\u00f4mes tels que l\u2019irritation des yeux, des maux de t\u00eate, le mal de gorge, la toux et des \u00e9coulements nasaux\u00a0(1,6). Certaines personnes sont plus vuln\u00e9rables que d\u2019autres aux effets de l\u2019exposition \u00e0 la fum\u00e9e d\u2019incendie forestier sur leur sant\u00e9, y compris les personnes \u00e2g\u00e9es, celles qui travaillent \u00e0 l\u2019ext\u00e9rieur, les enfants, les femmes enceintes et les personnes souffrant d\u00e9j\u00e0 de probl\u00e8mes de sant\u00e9\u00a0(1,6). L\u2019exposition peut avoir pour effet d\u2019aggraver les maladies cardiovasculaires et pulmonaires. En effet, elle est associ\u00e9e \u00e0 un taux de mortalit\u00e9 plus \u00e9lev\u00e9 et \u00e0 un plus grande morbidit\u00e9 respiratoire\u00a0(7,8). Les effets \u00e0 long terme d\u2019expositions aigu\u00ebs et p\u00e9riodiques \u00e0 la fum\u00e9e des feux de for\u00eats sont toujours inconnus et constituent un aspect important de la recherche \u00e9mergente, et la saison des feux de for\u00eat au Canada s\u2019allonge de plus en plus et ces incendies se font de plus en plus intenses\u00a0(9\u201111). Selon le Centre international de recherche sur le cancer, la pollution de l\u2019air ext\u00e9rieur et les particules associ\u00e9es seraient canc\u00e9rog\u00e8nes pour l\u2019homme et la fum\u00e9e de feux incontr\u00f4l\u00e9s contribuerait \u00e0 l\u2019effet canc\u00e9rog\u00e8ne\u00a0(12).<\/p>\n<p>Certains travailleurs peuvent \u00eatre plus expos\u00e9s \u00e0 la fum\u00e9e de feux incontr\u00f4l\u00e9s et \u00e0 ses effets sur la sant\u00e9. L\u2019exposition des pompiers forestiers, en particulier, est plus \u00e9lev\u00e9e que pour le public en g\u00e9n\u00e9ral en raison de leur proximit\u00e9 des feux, du volume d\u2019air aspir\u00e9 plus \u00e9lev\u00e9 et d\u2019une protection respiratoire parfois inad\u00e9quate\u00a0(13,14).
